This form includes the title and description of the invention along with details about publications and use. The form should be completed, signed by all inventors, and submitted to the ORPC early in the invention process.
Agreement to Disclose Inventions [.doc] [.pdf]
The Bayh-Dole Act obligates the University to require, by written agreement, each employee to disclose inventions which were discovered, created, and/or reduced to practice on federally funded research.
Participation Agreement [.pdf]
The purpose of this agreement is to assure that the inventors/authors/creators have read the Intellectual Property Policy and agree to assist the Office of the Vice President for Research and Public Service in the evaluation and protection of the intellectual property.
Confidentiality Agreement [.doc] [.pdf]
This agreement is between an outside company and UNH. The purpose of this agreement is to allow UNH to disclose information to a company for evaluation of technical feasibility, novelty, and potential commercial value.
Royalty Sharing Agreement [.doc] [.pdf]
This agreement indicates the division of royalties for more than one inventor, author and/or developer.
